SMS Terms & Conditions

These SMS Terms and Conditions ("SMS Terms") govern your participation in the My Reminder Box Caregiver SMS Alerts program (the "Program"). By opting in to receive SMS messages from My Reminder Box, you agree to these SMS Terms.

1. Program Description

The My Reminder Box Caregiver SMS Alerts program sends automated SMS text message notifications to enrolled caregivers when a scheduled medication reminder for a designated patient has not been confirmed within a set time window. The Program is operated by My Reminder Box ("we," "us," or "our") at myreminderbox.com.

Messages may include: notification that a scheduled medication reminder was not confirmed, reminders to check on your loved one, and occasional program-related service messages.

2. How to Opt In

To enroll in the Program, you must complete the Caregiver SMS Alerts sign-up form at myreminderbox.com/caregiver-sms-alerts and check the SMS consent checkbox confirming your agreement to receive automated text messages. By completing this form and submitting it, you expressly consent to receive SMS messages from My Reminder Box at the mobile phone number you provide.

You must be the authorized account holder or have the permission of the authorized account holder for the mobile phone number you provide. You must be 18 years of age or older to enroll.

3. Message Frequency

Message frequency varies. The number of SMS messages you receive depends on the patient's medication schedule and how often reminders go unconfirmed. You may receive multiple messages per day if multiple medications are scheduled and unconfirmed. You will also receive a one-time confirmation message when you opt in, and a one-time confirmation message when you opt out.

4. Message and Data Rates

Message and data rates may apply. The cost of receiving SMS messages depends on your mobile carrier and your wireless plan. My Reminder Box does not charge you for SMS messages, but your mobile carrier may charge you for sending and receiving text messages. Please check with your carrier if you are unsure about your plan's messaging rates.

5. How to Opt Out (STOP)

You may opt out of the Program at any time. To stop receiving SMS messages from My Reminder Box, reply STOP to any SMS message you receive from us. After we receive your STOP request, we will send you a one-time confirmation message and you will no longer receive SMS messages from the Program.

You may also contact us directly at [email protected] to request removal from the Program.

If you opt out, you will no longer receive caregiver SMS alerts. To re-enroll, you must complete the sign-up form again.

6. How to Get Help (HELP)

If you need assistance, reply HELP to any SMS message you receive from us. You will receive a response with contact information and instructions. You may also contact us directly:

7. Supported Carriers

My Reminder Box SMS messages are supported by most major US wireless carriers, including but not limited to:

  • AT&T
  • Verizon Wireless
  • T-Mobile
  • Sprint (T-Mobile)
  • US Cellular
  • Boost Mobile
  • Cricket Wireless
  • Metro by T-Mobile
  • Other regional and prepaid carriers

Carriers are not liable for delayed or undelivered messages. Message delivery is not guaranteed and may be affected by your carrier, device, or network conditions.

8. Privacy

Your mobile phone number, SMS opt-in data, and messaging consent are used only to provide My Reminder Box services and the medication-status notifications you requested. We do not sell, rent, share, or disclose mobile phone numbers, SMS opt-in data, or messaging consent to third parties or affiliates for marketing or promotional purposes. 9. Changes to These Terms

We reserve the right to modify these SMS Terms at any time. Changes will be effective when posted to this page with an updated effective date. Your continued participation in the Program after changes are posted constitutes your acceptance of the updated terms. We may also notify you of material changes via SMS.
